Preserve Breast Augmentation in Dubai

Preserve Breast Augmentation in Dubai

At Seline Clinic in Jumeirah, Dubai, Preservé breast augmentation is performed by Consultant Plastic Surgeon Dr. Leonard Nenad Josipovic. Suitability is assessed individually, based on your breast anatomy, tissue quality, desired volume and long-term goals. The consultation decides whether Preservé is the right pathway, or whether another breast procedure would serve you better.

This page explains how Preservé works, who it suits, recovery, risks and cost.

What Is Preservé Breast Augmentation?

Preservé is a way of creating the space for a breast implant with less conventional cutting of the breast tissue. Instead of opening a pocket by dissection, the surgeon uses a set of dedicated Motiva instruments to access and gently expand the implant space.

The approach was developed by Establishment Labs, the manufacturer of Motiva implants. It uses Motiva Ergonomix2 implants, placed through a small incision in the fold beneath the breast.

Preservé belongs to a wider group of tissue-preserving breast techniques supported by early clinical evidence. It is not suitable for every patient, and its value depends on whether your anatomy and volume goals match what the technique can achieve.

What Can Preserve Do, and What Can It Not Do?

Preserve can add breast volume in suitable patients while limiting disruption to the surrounding tissue. It does not lift a breast that has dropped.

What the approach is designed to do:

  • Limit tissue disruption: less interference with gland, supporting ligaments and tissue planes
  • Shape the pocket gently: controlled tissue stretching rather than sharp or blunt dissection
  • Protect the implant during placement: an insertion sleeve reduces direct handling and contamination risk
  • Keep the chest muscle intact: the implant is typically placed above the muscle (prepectoral)
  • Support recovery: some selected patients may return to light activity sooner, although recovery varies

What it cannot do:

  • Lift sagging breasts: a low nipple position needs a breast lift
  • Suit very large size changes: the instruments are designed for a defined size range
  • Replace adequate tissue cover: thin tissue can make implant edges more visible
  • Remove all risk: it is still breast implant surgery

A 2026 retrospective study of 111 consecutive patients, published in the Aesthetic Surgery Journal, reported favourable outcomes at 12 months with Preservé instruments. The authors describe tissue preservation as an evolving field with promising early data, not proof that it is better for everyone.

Preserving tissue is a principle. Choosing the right patient is what makes it work.

Who May Be a Good Candidate for Preserve?

Preservé usually suits healthy women seeking a first breast augmentation who have enough breast tissue to cover the implant and little or no sagging.

Preservé may be considered if you:

  • Are having breast augmentation for the first time
  • Have adequate breast tissue and skin to cover and support the implant
  • Have limited or no breast sagging (ptosis)
  • Have a realistic volume goal for your chest and tissue dimensions
  • Are in stable general health, with a breast health assessment before surgery
  • Are not pregnant or breastfeeding and have no immediate plans for pregnancy

Candidacy is confirmed at consultation, after examination.

When Preservé May Not Be the Right Choice on Its Own

Some situations call for a different or combined plan:

  • Significant sagging: augmentation alone does not reposition the breast; a lift, with or without an implant, may be needed
  • A large volume change: very large increases may fall outside what the technique is designed for
  • Thin soft tissue: limited cover raises the risk of visible implant edges or rippling
  • Complex asymmetry: marked differences in size or position may need a more individual plan
  • Previous implant problems: capsular contracture or malposition usually needs a revision strategy
  • Unresolved breast findings: any lump or imaging concern is assessed before elective surgery
  • Unstable weight or near-term pregnancy plans: stability is generally advised first

How Preserve Works

Preserve follows a set sequence: assessment, controlled access, gentle pocket formation, protected implant placement and closure.

Breast and Implant Assessment

Dr. Leonard measures chest width and breast base, assesses breast volume, tissue thickness, skin quality, nipple position, the breast fold and any asymmetry, and discusses your goals. Implant sizing is guided by your anatomy, with proportion as the aim. This stage also confirms whether Preservé is the right technique.

Controlled Access: the Channel Separator

A single-use Channel Separator creates a pathway to the implant space through natural tissue planes, rather than opening tissue with blades or scissors. The breast's supporting structures are disturbed as little as the technique allows.

Pocket Formation: the Inflatable Balloon

An adjustable balloon is placed and gently expanded to form the implant pocket by stretching the tissue. Its size can be adjusted, so the pocket is shaped in a controlled way.

Implant Placement: Ergonomix2 and the Insertion Sleeve

The Motiva Ergonomix2 implant is delivered through an insertion sleeve, which limits handling of the implant shell and surface contamination. Position and symmetry are checked before closure.

Closure and Follow-Up

The incision in the fold beneath the breast is closed with sutures. Scar length depends on implant size and anatomy. A supportive garment may be applied, and follow-up is arranged at Seline Clinic.

Motiva Ergonomix2: The Implant Used With Preserve

Preservé uses the Motiva Ergonomix2 implant, a smooth-surface silicone gel implant that combines several design features.

  • SmoothSilk surface: a smooth, nano-textured shell designed to reduce friction against surrounding tissue
  • ProgressiveGel Ultima: a cohesive gel that holds shape at rest and moves more softly with motion
  • True Monobloc+ construction: shell, gel and fill port as a single unit, without a separate patch
  • BluSeal+ barrier: an extra layer designed to reduce gradual gel bleed through the shell
  • Zen RFID chip: a passive identification chip that allows the implant to be identified without surgery

Preservé vs Traditional Breast Augmentation

Traditional breast augmentation is a well-established procedure with decades of safety data. Preservé is a different technical approach for selected patients, and neither is better for everyone.

The main differences:

  • Pocket creation: Preservé uses a channel separator and balloon; traditional surgery uses manual dissection
  • Implant: Preservé uses Motiva Ergonomix2; traditional augmentation can use several implant types
  • Muscle: Preservé is typically above the muscle; traditional placement may be above or below it
  • Incision: Preservé uses the breast fold; traditional surgery may use the fold, the areola edge or the armpit
  • Anaesthesia: Preservé may use sedation with local anaesthesia where appropriate, or general anaesthesia when indicated
  • Suitability: Preservé suits selected patients; traditional augmentation covers a broader range, including combined lifts

The right choice depends on your anatomy, goals and tissue quality.

Other Breast Procedures at Seline

If Preservé is not the right fit, the consultation can point to another option:

  • Breast lift: for breasts that have dropped; it reshapes and raises the breast but does not add volume on its own
  • Breast augmentation with lift: for women with both volume loss and sagging, where anatomy allows
  • Fat transfer to the breast: uses your own fat for a more modest increase without an implant, depending on available donor fat
  • Areola reduction: to reduce the size of the areola, alone or with other breast surgery
  • Revision surgery: for problems with existing implants, assessed individually

Your Preservé Consultation at Seline Clinic

The consultation is a clinical assessment, not a commitment to surgery. Dr. Leonard uses it to explain what suits your anatomy before any decision is made.

  • Your goals: the change you hope for, what proportion means to you, and your concerns about surgery and recovery
  • Breast examination: tissue cover, skin quality, nipple position, the breast fold, chest dimensions and asymmetry
  • Breast health review: medical history, breast symptoms, imaging, relevant family history, medicines, pregnancy and breastfeeding
  • Implant planning: volume, base width and projection, guided by measurements rather than cup size
  • Technique decision: whether Preservé is appropriate, or whether a lift or combined plan would serve you better
  • Informed consent: what the procedure involves, realistic results, recovery, risks and alternatives

Smoking, nicotine, blood-thinning medication and supplements are discussed before surgery. Never stop a prescribed medicine without speaking to the doctor who prescribed it.

Risks, Safety and Limitations

Preservé's tissue-preserving approach does not make breast surgery risk-free. Bruising, swelling, tenderness and tightness are expected parts of healing.

Surgical risks include infection, bleeding, haematoma, seroma, wound healing problems, asymmetry and scarring, as well as anaesthesia complications and, rarely, blood clots. Implant-related risks include malposition, capsular contracture, rupture, visible rippling and dissatisfaction with size or shape.

Changes in nipple or breast sensation can occur after any implant surgery, including Preservé; most improve over months, but some may persist. Some patients need revision surgery for medical or aesthetic reasons. All risks are discussed in full, and in writing, before consent.

Breast Sensation, Breastfeeding and Pregnancy

Preservé aims to limit interference with breast tissue, but it cannot guarantee unchanged sensation or the ability to breastfeed.

  • Sensation: changes can occur, as after any breast surgery; most improve over months
  • Breastfeeding: depends on individual anatomy and factors unrelated to surgery; discuss it if it matters to you
  • Pregnancy: pregnancy changes breast volume, shape and skin, so completing planned pregnancies first is usually advised

Elective cosmetic surgery is postponed during pregnancy. After childbirth, timing is individualised according to recovery, breastfeeding status, medications, anaesthesia considerations and overall health.

Mammograms and Breast Imaging After Implants

Women with breast implants still need appropriate breast screening. Tell the imaging team that you have implants, so the technique can be adapted with extra views or displacement methods.

Ultrasound or MRI may be used to check implant integrity or the surrounding tissue. Any new lump, skin change, pain or asymmetry should be assessed clinically rather than assumed to be implant-related. Seline provides written implant identification details, including the Zen RFID chip information.

How Long Do Preservé Results Last?

Breast implants are long-lasting medical devices, but they are not permanent. Your breasts continue to change with age, weight, hormones and pregnancy, with or without an implant.

Routine breast monitoring continues after augmentation, and future revision may be needed at some point. An implant is a decision for today and a commitment to follow-up later.

Preserve Breast Augmentation Cost in Dubai

Preservé breast augmentation at Seline Clinic starts from AED 40,000 for primary augmentation in suitable patients. This covers the surgeon's fee, Motiva Ergonomix2 implants, the facility and anaesthesia.

The final cost depends on:

  • Implant selection: size and projection
  • Plan: augmentation alone, or combined with a lift or another procedure
  • Complexity: individual anatomy and asymmetry
  • Assessments and aftercare: pre-operative checks and follow-up

A detailed, itemised quotation is provided after your consultation. Prices are indicative starting prices and can change. The final surgical plan and quotation are confirmed after medical assessment.

What Happens on the Day of Surgery?

Surgery takes place in an accredited, DHA-licensed surgical facility, and Dr. Leonard performs the procedure. Consultation and follow-up care are provided at Seline Clinic on Jumeirah Street.

  • Anaesthesia: agreed in advance; sedation with local anaesthesia where clinically appropriate, or general anaesthesia when indicated
  • Surgical steps: channel separator for access, balloon for the pocket, insertion sleeve for the implant
  • Incision: in the fold beneath the breast, closed with sutures
  • Afterwards: monitoring before discharge; a responsible adult should take you home and stay with you the first night

Recovery After Preservé

Recovery is individual, and Dr. Leonard gives specific instructions for your case. Most patients feel pressure, tenderness and swelling in the first days, with gradual return to normal activity over the following weeks.

  • First days: rest, prescribed pain relief, no driving, and wound care as instructed
  • First week: light activity at home; wear the supportive bra; avoid lifting, reaching overhead and strenuous activity; first review appointment
  • Following weeks: activity increases gradually after follow-up; scar care starts once wounds have healed
  • Longer term: swelling settles and the implants soften into position over weeks to months; the final result is assessed from around six months

Dubai's sun and heat matter during recovery. Keep the scar out of direct sun and use sun protection consistently while it matures.

Contact the clinic if you notice:

  • Increasing warmth, redness or swelling after the first few days
  • Fever or unusual discharge from the incision
  • One breast becoming suddenly larger, tense or painful
  • Shortness of breath, chest pain or a painful swollen calf

Scars After Preservé

Preservé uses an incision in the natural fold beneath the breast, where a well-healed scar is usually hidden when standing. It is not a scarless procedure.

Scar quality depends on healing, genetics, skin type, sun exposure and aftercare. Silicone scar products and sun protection are commonly advised, and scars mature over 12 to 18 months.

I had my upper and lower blepharoplasty at Seline Clinic, and the experience was outstanding from start to finish. Dr. Leonard explained everything clearly and made me feel calm and confident before the surgery. The recovery was much easier than I expected, and the result looks incredibly natural — no signs of surgery, just a fresher, more rested look. The team was caring and professional throughout the whole process.

I recently had my breast augmentation with Dr. Alexej Rubin at Seline Clinic, and it was an amazing experience. He took time to understand exactly what I wanted and guided me to the perfect size and shape. The recovery was smooth, and I felt looked after every step of the way. The results are beautiful — soft, natural, and perfectly balanced. A truly five-star experience with a wonderful team.

No. Suitability depends on anatomy, tissue quality, skin elasticity, breast position, desired volume and health. Significant sagging, thin tissue, large volume goals or complex breast conditions may point to a different approach.
